This form is used only to submit Competitive game sheets. Teams are to submit game sheets for all of their games using this form including all regular season, playoff, exhibition, and tournament games. For league games the home team is responsible for the submission. Scanned copies in PDF format are preferred but you may also take a photo for submission. Please make sure that images of the game sheet are clear. One image must be of the game sheet in its entirety, but you may also consider taking closer image of each the home and away sides of the sheet.
